Finding a PATH® back home

Clay Center resident Patricia Woodruff thought she was simply stepping out of the car last Mother’s Day - but in an instant she fell and broke her ankle in three places.

That’s how she ended up spending several months at Clay Center Presbyterian Manor, through our PATH® (Post-Acute to Home) program.

“They were really helpful in getting me back home,” Patricia said. “They were very experienced and very good.”

With PATH® , our health care professionals tailor therapies and skilled nursing care to meet each patient’s needs. The PATH® program includes physical, occupational, and speech therapy as well as offering orthopedic, post-cardiac, and neurological and stroke rehab.

Patricia said it was nice to be in a place with such great care - where she didn’t need to worry about the daily chores or navigating her home with an injury.

Now that she’s back home, she’s getting healthier every day.

“I still go to the hospital for physical therapy,” she said. “I graduated to a cane from a walker.”
